New Customer Account Setup
To set up your account with us we would need you to complete the following forms and requirements:
- Completed customer request form.
- W9 Form / W8 within US/ GST-HST for CA / for International any local government document supporting legal name
- Tax exempt resale certificate
- Desired payment terms form.
- Withholding tax certificate
